The GoodBye Chain Group

When requesting a declaration for Conflict Minerals status, GCG is mandated to collect the standard EICC-GeSI form to satisfy requirements for its clients to report to the Securities and Exchange Commission under the Dodd-Frank Wall Street Reform and Consumer Protection Act, signed in to law on July 21, 2010. This is also known as the "Conflict Minerals Law".

.

This form includes a Conflict Minerals Reporting Template, used to determine and disclose the chain of custody of the Conflict Minerals in your products and your company’s sourcing practices of Conflict Minerals. This form is part of the EICC Conflict Free Smelter Program and satisfies the reasonable country of origin inquiry required by the legislation.

We request that this form be completed in full and emailed to us. Link to the form, including an instructional video, may be accessed through EICC's Conflict Minerals Reporting Template and Dashboard, located from the following page: http://www.conflictfreesourcing.org/conflict-minerals-reporting-template/.

If you have further questions regarding how to complete the EICC-GeSI form, we offer a 15 minute online training session to guide suppliers through the process. Training requests may be emailed to our Data Collection Support Team at This email address is being protected from spambots. You need JavaScript enabled to view it.

Providing the complete EICC-GeSI Form will eliminate the need for repeat requests for an alternate written declaration for the calendar year in which reporting must be completed.
